The Studies on the Differences in Drinking Habits by Aldehyde Dehydrogenase-I Genotype Variances in the Korean Male Alcoholics

Abstract
Since it has been supposed that unpleasant flushing reaction in Orientals after alcohol ingestion is probably arised from the deficiency of aldehyde dehydrogenase-I (ALDH-I), one of isozyme working on the 2nd step of alcohol metabolism, and known
the
fact that deficient rate of ALDH-I is focused as the biological markers in alcoholism. Namely the deficient enzymatic activity of ALDH-I results in the accumuation of acetaldehyde that is probable cause of unpleasant flushing reaction. Therefore
the
deficients of this isozymes are inclined to be aversive to alcoholic beverage, and have less cnance of being an alcholic than in the normal ones who have ALDH-I. On the contrary, the other research workers have asserted that accumulation of
acetaldehyde
combined with biogenic amines can produce morphin-like addicting substances and reinforce alcohol dependence. But the research up to now has not been carried in detail especially in the alcoholics. So the author carried out the detailed study to
elucidate the relationships between ALDH-I genotype and drinking habits and clinical history in 102 admitted Korean adult male primary alcoholics. In this study, we analysed the ALDH-I genotype and as a result, NN type is 84.4%, ND type is 15.6%
and
there are no DD type patients. So evidently if compared with other study in general population, phenotypically ALDH-I deficient rate is significantly lower in alcoholics. And in drinking habit comparison by ALDH-I Genotype, ND group compared to
NN
group
has more common withdrawl symptoms and physical diseases induced by alcohol ; less magnitudes of sips : longer interval between sips ; later onset of alcoholism, more common life event stress at alcoholism onset, and seem to be soon aggravated.
So
we
can assume that those who have ND type ALDH-I, so called Oriental type, are less likely to be alcoholics : but once they become alcoholics by precipitating factors like life event stresses, they are more likely to have severe dependence and soon
aggravating clinical courses.
